Background: Hospital-acquired infections (HAIs) caused by multidrug resistant (MDR) bacteria are a major public health problem especially in resource-limited settings. Local data on antimicrobial resistance (AMR) are crucial to inform empirical therapy and infection prevention. Objectives: To identify bacterial pathogens causing HAIs, determine their AMR profiles and independent risk factors for MDR infections in a tertiary hospital in Erbil City, Iraq. Materials and Methods: A cross-sectional study was done at Hawler Teaching Hospital between January and June 2026. Standard microbiological methods were used to analyse 200 clinical specimens from patients with HAIs (≥48 hr after admission). Antimicrobial susceptibility testing was performed according to CLSI 2024 guidelines. MDR was defined as nonsusceptibility to at least one agent in three or more antimicrobial classes. Multivariate logistic regression was used to assess independent risk factors. Results: Bacterial growth was obtained in 149 of 200 specimens (74.5%). The major pathogens were Escherichia coli (31.5%), Klebsiella pneumoniae (23.5%), Staphylococcus aureus (21.5%), Pseudomonas aeruginosa (14.1%) and Acinetobacter baumannii (6.1%). In total, 69.1% of the isolates were MDR, with the highest rates in K. pneumoniae (80.0%) and A. baumannii (77.8%). Independent predictors of MDR infection were previous antibiotic use (AOR=4.45), ICU admission (AOR=3.92), and hospitalization >7 days (AOR=3.10). Conclusion: This study documented a significant burden of MDR-HAIs in Erbil City and determined modifiable clinical risk factors. The findings provide evidence to support antimicrobial stewardship, infection prevention and AMR surveillance in Iraqi healthcare settings.

This study presents a physics-based parametric optimization framework for thermoelastic energy harvesting from cyclic temperature variations using a cylindrical AISI 4340 steel rod. The model couples thermoelastic force and displacement, Euler buckling and yielding constraints, lumped-capacitance heat transfer, and mass-power multi-objective optimization. The feasible design space was evaluated as a function of rod diameter, length, temperature change, and load-transfer parameter. The results show that Euler buckling governs the practically relevant portion of the design space at moderate temperature changes, whereas yielding becomes increasingly restrictive at higher thermal loads. The Pareto-optimal solutions exhibit a clear mass-power scaling law, with the analytical prediction P_avg ∝ m^2/3 closely reproduced by the numerical fit (n = 0.66717, R² = 0.999999). Under the baseline condition λ = 0.50, the maximum-power design produces approximately 0.16624 W, while a distance-to-ideal criterion identifies a substantially lighter compromise design with m = 223.0 kg and P_avg = 0.09231 W. Refined optimization gives λ_opt = 0.473 and P_max = 0.170454 W, indicating a power penalty of only 2.47% when the simpler λ = 0.50 condition is used. Grid-refinement tests show changes below 0.006% in maximum power and below 0.5% in the compromise-design mass between the 250 × 250 and 500 × 500 grids. The results establish compact scaling and optimization rules for balancing structural stability, thermal response, mass, and average mechanical power in thermoelastic energy-harvesting elements.

Background: Morris syndrome, also known as androgen insensitivity syndrome (AIS), is a disorder of sex development caused by mutations in the androgen receptor gene in individuals with a 46,XY karyotype. The syndrome is typically diagnosed during adolescence due to primary amenorrhea, and early detection in childhood is uncommon. Prompt recognition is clinically important to guide multidisciplinary management and reduce the risk of gonadal malignancy. Case Description: We report the case of a one-year-old phenotypic female presenting with an irreducible, painful right inguinal hernia. Ultrasound suggested bilateral inguinal hernias containing ovary-like structures. During emergency surgical exploration, bilateral testes were unexpectedly identified within the inguinal canals. The testes were reduced into the abdominal cavity, and herniotomy with anterior wall repair was performed bilaterally. External genital examination revealed normally developed labia but absence of the hymenal ring and vaginal opening. The postoperative course was uneventful. Despite referral for karyotype analysis and multidisciplinary follow-up, the parents did not return for further evaluation. Conclusion: This case highlights the rare intraoperative diagnosis of Morris syndrome in early childhood. Awareness of AIS is essential for pediatric surgeons managing inguinal hernias in phenotypic females. Early detection allows timely planning of gonadectomy and long-term multidisciplinary management, minimizing the risk of malignant transformation of undescended testes and optimizing patient outcomes.

Objective: This systematic review aimed to identify, synthesize, and critically analyze the available evidence on clinical protocols used for the prevention and treatment of prosthetic joint infection (PJI) in total hip arthroplasty (THA), based on studies published between 2000 and 2025. Methods: The review was conducted according to PRISMA guidelines. Electronic searches were performed in PubMed (MEDLINE), Scopus, Web of Science, and Embase between January and April 2025. Eligible studies included clinical trials, cohort studies, case-control studies, systematic reviews, and meta-analyses published in English that addressed either preventive or therapeutic strategies for PJI in THA. Study selection, data extraction, and quality assessment were carried out independently by two reviewers. Due to the heterogeneity of the included studies, a qualitative synthesis was performed. Results: A total of 32 studies were included. Preventive measures identified in the literature comprised combined antibiotic prophylaxis (cefazolin and gentamicin), multimodal perioperative protocols such as ACERTO, nasal decolonization for Staphylococcus aureus, silver-impregnated dressings, and structured post-discharge surveillance. Treatment strategies included DAIR (Debridement, Antibiotics, and Implant Retention), the DAPRI technique, one-stage and two-stage revision surgeries, muscle flap reconstructions, and protocols without spacers. These interventions were associated with significantly reduced infection rates and improved clinical outcomes when applied appropriately and in accordance with patient-specific factors. Conclusion: Effective prevention and treatment of PJI in total hip arthroplasty require a systematic and evidence-based approach. Integrated protocols—spanning preoperative optimization, meticulous intraoperative techniques, and rigorous postoperative monitoring—have proven effective in reducing infection incidence. In cases of established infection, surgical management must be tailored to the timing of infection, microbial profile, and host conditions. Two-stage revision remains the gold standard for complex infections, while one-stage revision and emerging techniques like DAPRI offer promising results in selected cases. This review contributes to the standardization of clinical practice and supports improved patient outcomes.

This research is a collaboration involving a university team, a partnering corporation, and a hemodialysis clinic, which is a cross-disciplinary research initiative in the field of Artificial Intelligence of Things (AIoT) within the medical informatics domain. The research has two objectives: (1) The development of an Internet of Things (IoT)-based Information System customized for the hemodialysis machines at the clinic, including transmission bridges, clinical personnel dedicated web/app, and a backend server. The system has been deployed at the clinic and is now officially operational; (2) The research also utilized de-identified, anonymous data (collected by the officially operational system) to train, evaluate, and compare Deep Learning-based Intradialytic Blood Pressure (BP)/Pulse Rate (PR) Predictive Models, with subsequent suggestions provided. Both objectives were executed under the supervision of the Institutional Review Board (IRB) at Mackay Memorial Hospital in Taiwan. The system completed for objective one has introduced three significant services to the clinic, including automated hemodialysis data collection, digitized data storage, and an information-rich human-machine interface as well as graphical data displays, which replaces traditional paper-based clinical administrative operations, thereby enhancing healthcare efficiency. The graphical data presented through web and app interfaces aids in real-time, intuitive comprehension of the patients’ conditions during hemodialysis. Moreover, the data stored in the backend database is available for physicians to conduct relevant analyses, unearth insights into medical practices, and provide precise medical care for individual patients. The training and evaluation of the predictive models for objective two, along with related comparisons, analyses, and recommendations, suggest that in situations with limited computational resources and data, an Artificial Neural Network (ANN) model with six hidden layers, SELU activation function, and a focus on artery-related features can be employed for hourly intradialytic BP/PR prediction tasks. It is believed that this contributes to the collaborating clinic and relevant research communities.

This article presents a mathematical model designed to simulate the impact of targeted interventions aimed at preventing HIV transmission among female sex workers (FSWs) and their clients, while also analyzing their effects on the health of the general population. The compartmental model distinguishes between high-risk populations (FSWs and their clients) and low-risk populations (sexually active men and women in the general population), and links prevention efforts in high-risk groups to the evolution of the epidemic in the low-risk population. The fundamental properties of the model, such as the positivity of solutions and the boundedness of the system, have been verified, and the basic reproduction number R₀ has been calculated. Finally, the stability of the model was studied using Varga’s theorem and the Lyapunov method. Simulation results show that targeted prevention among FSWs and their clients reduces HIV incidence in the general population. This framework provides a valuable tool for guiding policymakers in the design of effective strategies to combat the epidemic, especially relevant in the context of suspension of USAID funding.

The integration of cloud-based ERP solutions, the incorporation of emerging technologies like Artificial Intelligence (AI), Machine Learning (ML), and the Internet of Things (IoT), and enhanced data analytics capabilities have revolutionized pharmaceutical IT. These advancements not only reduce operational costs, improve forecasting accuracy, and enhance collaboration but also ensure compliance with stringent global regulations, such as Good Manufacturing Practices (GMP) and FDA guidelines. Moreover, ERP systems have been instrumental in managing the pharmaceutical supply chain, ensuring product traceability, and improving inventory control and order fulfillment processes. This manuscript examines how ERP systems enable pharmaceutical companies to maintain high standards of product quality, improve decision-making, and ensure the safety and efficacy of drugs through robust tracking and auditing mechanisms. A case study of a pharmaceutical company that implemented an ERP system demonstrates the tangible benefits, including increased operational efficiency, improved compliance rates, and enhanced customer satisfaction. However, despite the clear advantages, challenges such as customization complexities, data integration issues, and resistance to change remain. As the pharmaceutical industry continues to evolve, ERP systems will remain a cornerstone of digital transformation, facilitating smarter decision-making, better resource management, and enhanced collaboration across global operations. This paper also identifies future trends, including the potential of AI and blockchain technologies in further strengthening ERP systems and transforming the pharmaceutical landscape.

Preference learning plays a crucial role in AI applications, particularly in recommender systems and personalized services. Traditional pairwise comparisons, while foundational, present scalability challenges in large-scale systems. This study explores alternative elicitation methods such as ranking, numerical ratings, and natural language feedback, alongside a novel hybrid framework that dynamically integrates these approaches. The proposed methods demonstrate improved efficiency, reduced cognitive load, and enhanced accuracy. Results from simulated user studies reveal that hybrid approaches outperform traditional methods, achieving a 40% reduction in user effort while maintaining high predictive accuracy. These findings open pathways for deploying user-centric, scalable preference learning systems in dynamic environments.

Serialization involves assigning unique identifiers to individual drug packages, enabling precise tracking and authentication at every stage of the supply chain. This process provides unprecedented transparency, enhances product security, and facilitates real-time monitoring of pharmaceutical products as they move from manufacturers to end consumers. Despite its potential to revolutionize pharmaceutical traceability, the integration of serialization technologies faces numerous obstacles. These include high implementation costs, regulatory inconsistencies across regions, and the technological challenges of managing vast amounts of data. Moreover, the complex, multi-tiered nature of the global supply chain introduces additional risks related to data integrity, cybersecurity, and interoperability between systems. As pharmaceutical companies seek to navigate these challenges, innovations in serialization technology—such as blockchain, artificial intelligence (AI), the Internet of Things (IoT), and radio frequency identification (RFID)—are providing promising solutions to enhance efficiency, reduce fraud, and increase visibility. This manuscript explores both the innovative advancements and the key challenges associated with the integration of serialization in the pharmaceutical supply chain. It delves into the evolving regulatory landscape, highlighting the need for global harmonization of serialization standards, and examines the impact of serialization on securing pharmaceutical distribution networks. Additionally, the paper emphasizes the importance of collaboration among manufacturers, technology providers, and regulatory bodies in overcoming implementation barriers and realizing the full potential of serialization. As the pharmaceutical industry moves towards a more interconnected and data-driven future, serialization promises to play a central role in shaping the next generation of drug safety and supply chain management. By addressing the hurdles to adoption and leveraging emerging technologies, the pharmaceutical sector can create a more secure, transparent, and efficient supply chain that better serves public health and fosters greater trust among consumers and healthcare professionals alike.

Background: Educational attainment is generally associated with reduced reliance on Social Security and disability benefits; however, the Minorities' Diminished Returns (MDRs) theory suggests that the socioeconomic benefits of education are weaker for minoritized populations. This study investigates the relationship between educational attainment and welfare receipt among American Indian/Alaska Native (AIAN) and White adults in the United States. Objective: Using the MDRs framework, we analyzed data from the National Health Interview Survey (NHIS) 2023 to examine how educational attainment impacts welfare receipt among AIAN and White adults. Methods: We analyzed a nationally representative sample of AIAN and White adults from the NHIS 2023 dataset. Welfare receipt was assessed as the receipt of any public assistance or welfare payments from state or local welfare offices. Educational attainment was categorized into three levels: less than high school (reference), high school diploma to some college, and college degree or higher. Logistic regression models were used to assess the relationship between educational attainment and welfare receipt, with separate analyses for AIAN and White adults to evaluate differential effects. Results: Higher educational attainment (high school diploma to some college and college degree or higher) was associated with lower odds of welfare receipt across both groups. However, the protective effect of a college degree was significantly weaker for AIAN adults compared to White adults. Consequently, AIAN adults remain at a higher risk of welfare reliance even with higher education, consistent with the Minorities' Diminished Returns (MDRs) framework. Conclusions: Although educational attainment generally reduces welfare reliance, this protection is less pronounced for AIAN adults than for White adults. This discrepancy suggests that structural factors, segregation, and social stratification may undermine the economic and health benefits of education for racialized groups in the U.S. Addressing these disparities requires policy interventions that extend beyond education, emphasizing quality job opportunities, healthcare access, and reduced labor market discrimination for individuals with advanced educational credentials, regardless of race.

Digital therapeutics (DTx) are emerging as a pivotal tool in promoting longevity by addressing non-communicable diseases (NCDs) such as diabetes, cardiovascular diseases, and mental health disorders. These software-driven interventions offer personalized, evidence-based treatments that can be accessed via digital devices, making healthcare more accessible and scalable. One of the key advancements in DTx is the integration of artificial intelligence (AI) and machine learning (ML) to tailor interventions based on individual health data. This personalization enhances the effectiveness of treatments and supports preventive care by identifying risk factors early. The need for digital therapeutics is underscored by the rising prevalence of NCDs, which are responsible for a significant portion of global mortality and healthcare costs. Traditional healthcare systems often struggle to provide timely and personalized care, especially in low-resource settings. DTx can bridge this gap by offering cost-effective solutions that are easily scalable. Moreover, digital therapeutics can address health inequities by providing low-cost interventions to underserved populations, thereby reducing the burden of NCDs and improving overall health outcomes. As technology continues to evolve, the potential for DTx to enhance longevity and quality of life becomes increasingly promising. Recent advancements in longevity medicine and technology have focused on extending both lifespan and healthspan, ensuring that people not only live longer but also maintain good health throughout their extended years. This review article highlights these advancements that are contributing to this compelling subject of Longevity.

Background: Research on Minorities’ Diminished Returns (MDRs) consistently reveals that social determinants of health, especially educational attainment, do not yield equal health benefits across racial and ethnic groups in the United States. MDRs suggest that social stratification, segregation, lower education quality, and labor market discrimination contribute to diminished health returns of education among minoritized groups. However, few studies have tested the relevance of MDRs in American Indian and Alaska Native (AIAN) populations compared to non-Hispanic White adults. Objectives: This study aimed to examine the strength of the inverse association between educational attainment and hypertension prevalence, hypothesizing that the protective effect of education on hypertension risk is reduced among AIAN adults relative to non-Hispanic Whites. Methods: Using data from the 2023 National Health Interview Survey (NHIS), we analyzed a nationally representative sample of adults aged 18 and older. Logistic regression models examined the association between educational attainment and self-reported hypertension diagnosis, stratified by racial/ethnic group (AIAN vs. non-Hispanic White). Models were adjusted for key covariates, including age, gender, income, and insurance status. Results: Higher educational attainment was associated with a lower prevalence of hypertension in the combined sample of AIAN and non-Hispanic White adults. However, this protective association was significantly weaker among AIAN adults compared to non-Hispanic White adults, as evidenced by a significant interaction between race and education. Conclusion: AIAN adults exhibit a higher prevalence of hypertension even at higher levels of educational attainment compared to non-Hispanic White adults, supporting the relevance of MDRs for AIAN populations. This finding underscores the need for public health interventions that address structural barriers and contextual factors unique to AIAN populations. Policies focused solely on educational access may be insufficient to reduce hypertension risk among AIAN adults without addressing broader social and structural inequities.

The integration of LiDAR and Artificial Intelligence (AI) has revolutionized feature detection in urban environments. LiDAR systems, which utilize pulsed laser emissions and reflection measurements, produce detailed 3D maps of urban landscapes. When combined with AI, this data enables accurate identification of urban features such as buildings, green spaces, and infrastructure. This synergy is crucial for enhancing urban development, environmental monitoring, and advancing smart city governance. LiDAR, known for its high-resolution 3D data capture capabilities, paired with AI, particularly deep learning algorithms, facilitates advanced analysis and interpretation of urban areas. This combination supports precise mapping, real-time monitoring, and predictive modeling of urban growth and infrastructure. For instance, AI can process LiDAR data to identify patterns and anomalies, aiding in traffic management, environmental oversight, and infrastructure maintenance. These advancements not only improve urban living conditions but also contribute to sustainable development by optimizing resource use and reducing environmental impacts. Furthermore, AI-enhanced LiDAR is pivotal in advancing autonomous navigation and sophisticated spatial analysis, marking a significant step forward in urban management and evaluation. The reviewed paper highlights the geometric properties of LiDAR data, derived from spatial point positioning, and underscores the effectiveness of machine learning algorithms in object extraction from point clouds. The study also covers concepts related to LiDAR imaging, feature selection methods, and the identification of outliers in LiDAR point clouds. Findings demonstrate that AI algorithms, especially deep learning models, excel in analyzing high-resolution 3D LiDAR data for accurate urban feature identification and classification. These models leverage extensive datasets to detect patterns and anomalies, improving the detection of buildings, roads, vegetation, and other elements. Automating feature extraction with AI minimizes the need for manual analysis, thereby enhancing urban planning and management efficiency. Additionally, AI methods continually improve with more data, leading to increasingly precise feature detection. The results indicate that the pulse emitted by continuous wave LiDAR sensors changes when encountering obstacles, causing discrepancies in measured physical parameters.

Artificial Immune Systems (AIS) are bio-inspired computational frameworks that emulate the adaptive mechanisms of the human immune system, such as self/non-self discrimination, clonal selection, and immune memory. These systems have demonstrated significant potential in addressing complex challenges across optimization, anomaly detection, and adaptive system control. This paper provides a comprehensive exploration of AIS applications in domains such as cybersecurity, resource allocation, and autonomous systems, highlighting the growing importance of hybrid AIS models. Recent advancements, including integrations with machine learning, quantum computing, and bioinformatics, are discussed as solutions to scalability, high-dimensional data processing, and efficiency challenges. Core algorithms, such as the Negative Selection Algorithm (NSA) and Clonal Selection Algorithm (CSA), are examined, along with limitations in interpretability and compatibility with emerging AI paradigms. The paper concludes by proposing future research directions, emphasizing scalable hybrid frameworks, quantum-inspired approaches, and real-time adaptive systems, underscoring AIS's transformative potential across diverse computational fields.

Time series data are increasingly prevalent across domains such as finance, healthcare, manufacturing, and IoT, making accurate forecasting and anomaly detection critical for decision-making and system reliability. Traditional statistical methods (e.g., ARIMA, Holt-Winters) often fail to capture complex temporal dependencies and high-dimensional interactions inherent in modern time series. Recent advances in artificial intelligence particularly deep learning architectures such as recurrent neural networks (RNNs), convolutional neural networks (CNNs), temporal convolutional networks (TCNs), graph neural networks (GNNs) and Transformers have demonstrated marked improvements in modeling both univariate and multivariate series, as well as in detecting anomalies that deviate from learned norms (Darban, Webb, Pan, Aggarwal, & Salehi, 2022; Chiranjeevi, Ramya, Balaji, Shashank, & Reddy, 2024) [1,2]. Moreover, ensemble techniques and hybrid signal-processing + deep-learning pipelines show enhanced sensitivity and adaptability in real-world anomaly detection scenarios (Iqbal, Amin, Alsubaei, & Alzahrani, 2024) [3]. In this work, we provide a unified survey and comparative analysis of AI-driven time series forecasting and anomaly detection methods, highlight key industrial application domains, evaluate performance trade-offs (e.g., accuracy vs. latency, supervised vs. unsupervised learning), and discuss emerging challenges including interpretability, data drift, real-time deployment on edge devices, and integration of causal reasoning. Our findings suggest that while AI approaches significantly outperform classical techniques in many settings, careful consideration of data characteristics, evaluation metrics and deployment environment remains essential for effective adoption.

High-Performance Computing (HPC) is indispensable for large-scale scientific simulations, but achieving optimal performance on modern supercomputers is increasingly challenging. As HPC systems scale toward exascale, they face escalating complexity in hardware, software, and workloads. Traditional optimization methods (manual tuning and heuristic algorithms) struggle to cope with dynamic workloads and intricate system behaviors. Artificial Intelligence (AI) techniques offer a promising approach to address these challenges. This article provides an overview of AI-powered optimization in HPC, focusing on how machine learning and related AI methods enhance performance, efficiency, and scalability of scientific simulations. We survey key AI techniques applied to HPC optimization including machine learning for performance modeling, deep reinforcement learning for resource management, and AI-driven surrogate models for accelerating simulations and illustrate their impact through case studies in domains such as job scheduling and fluid dynamics. We discuss the practical applications of these techniques, highlighting reported performance gains (e.g., substantial reductions in simulation run time and improved resource utilization). We also examine the challenges in integrating AI with HPC (such as training overhead, data movement, and reliability concerns) and outline future directions for research. The convergence of AI and HPC is poised to produce “smart” simulation workflows that intelligently adapt and optimize in real time, pushing the frontiers of scientific computing.

Pharmaceutical Product serialization aims to assign distinct serial numbers to items within a pharmaceutical supply chain. However, this process faces several security challenges like Theft of valid serial numbers may occur, enabling the labelling of counterfeit products. Therefore, it's essential to ensure the uniqueness of serial numbers can be verified at any point in the product's lifecycle within the supply chain. Intimidatory nodes along the distribution network could corrupt planned changes of custody for products. Ensuring verifiability of compliance with these changes is crucial. Manufacturers and consumers need assurance that perishable goods with expired shelf lives are appropriately discarded. In this paper, we review a product serialization method leveraging blockchain technology to address these security concerns within a multi-party perishable goods supply chain. Blockchains offer potential solutions by providing a secure platform for data sharing in multi-party environments, enhancing security and transparency. Within Blockchain technology, each distribution partner is registered to uphold transparency regarding drug information. The system facilitates real-time transfer of ownership changes, recording them as blocks with date and time stamps. This ensures visibility to all partners in real time, maintaining the authenticity of drugs. This article aims to outline how Blockchain technology benefits the pharmaceutical industry by enhancing traceability and trackability of drugs throughout the entire pharmaceutical supply chain.

PLCs, or programmable logic controllers, are essential parts of contemporary industrial automation systems and are responsible for managing and keeping an eye on a variety of operations. PLC reliability is critical to maintaining industrial systems' continuous and secure operation. A wide range of reliability strategies were used to improve the reliability of Programmable Logic Controllers, and this article methodically looks at them all. The evaluation classified PLC reliability techniques into Root Cause Analysis (RCA), Reliability Centered Maintenance (RCM), Hazard analysis (HA), Reliability block diagram (RBD), Fault tree analysis (FTA), Physics of failure (PoF) and FMEA/FMECA, after thoroughly reviewing the body of literature. Baloxavir marboxil (BXM) is a novel anti-influenza agent that developed in Japan and inhibit the cap endonuclease specifically, and suggested the more clinical effectiveness in influenza. BXM reduces viral shedding more than do neuraminidase inhibitors (NAIs), and its clinical efficacy is equivalent to that of NAIs but is superior to that of NAIs in the case of type B influenza. BXM does not demonstrate any issues with safety, and treatment with BXM reduces the incidences of complicating sinusitis and bronchitis. Prophylactic administration of BXM inhibits intrafamilial transmission of influenza although low susceptible viruses with a PA/I38X substitution are isolated with a certain frequency following administration of BXM. Finally, In influenza treatment for patients aged 12−19 years and for adult outpatients, BXM can be used with the same recommendation level as oseltamivir.
