The accurate estimation of Individual Wave Components (IWC) is crucial for automated diagnosis of the human digestive system in a clinical setting. However, this process can be challenging due to signal contamination by other signal sources in the body, such as the lungs and heart, as well as environmental noise. To address this issue, various denoising techniques are commonly employed in bowel sound signal processing. While denoising is important, it can increase computational complexity, making it challenging for portable devices. Therefore, signal processing algorithms often require a trade-off between fidelity and computational complexity. This study aims to evaluate an IWC parameter extraction algorithm that was previously developed and reconstruct the IWC without denoising using synthetic and clinical data. To that end, the role of a reliable model in creating synthetic data is paramount. The rigorous testing of the algorithm is limited by the availability of quality and quantity recorded data. To overcome this challenge, a mathematical model has been proposed to generate synthetic bowel sound data that can be used to test new algorithms. The proposed algorithm’s robust performance is evaluated using both synthetic and clinically recorded data. We perform time-frequency analysis of original and reconstructed bowel sound signals in various digestive system states and characterize the performance using Monte Carlo simulation when denoising is not applied. Overall, our study presents a promising algorithm for accurate IWC estimation that can be useful for predicting anomalies in the digestive system.

Anaphylaxis and Cardiogenic Pulmonary Edema due to Non ST Elevation Myocardial Infarction NSTEMI: A Case Report

Anaphylaxis can be associated with hemodynamic shock, which requires the early initiation of adrenaline as part of its management. Cardiogenic pulmonary edema is a frequent entity in emergency services with increased mortality in patients with acute coronary syndrome. The case report presents the case of a 55-year-old male patient who entered the emergency department with a non-ST-segment elevation myocardial infarction (NSTEMI) associated to pulmonary edema and anaphylaxis. During his stay in the emergency room, he had an anaphylactic reaction to dipyrone (metamizole) used for pain control. The patient presented signs of acute pulmonary edema, a hypertensive urgency after the use of adrenaline for the management of anaphylaxis.  There was doubt as to whether the dyspnea was of anaphylactic or cardiogenic origin, so an emergency ultrasound was performed, which suggested a bilateral pattern B.  This allowed timely management of ventilatory failure with systemic nitrates, diuretics, and oxygen therapy, which controlled blood pressure and resolved ventilatory failure. Subsequently, he was transferred to an institution with a hemodynamic service for the management of NSTEMI. We highlight the utility of emergency ultrasonography for immediate decision-making and the low prevalence of anaphylactic reaction in a patient with NSTEMI leading to acute pulmonary edema.

In this paper, we found a new model for a compact star with charged anisotropic matter distribution considering the generalized Chaplygin equation of state. The Einstein-Maxwell field equations have been solved with a particular form of metric potential and electric field intensity. The plots show that physical variables such as radial pressure, energy density, charge density, anisotropy, radial speed sound, and the mass are fully well defined and are regular in the star's interior. We obtained models consistent with stellar objects such as GJ 832, LHS 43, SAO 81292, GJ 380, GJ 412, and SAO 62377.

Meigs’ syndrome presenting with pleuritic chest pain and dyspnea: rapid resolution after resection of an ovarian fibroma

Meigs’ syndrome is a rare triad of a benign ovarian fibroma (or fibroma‑like tumor), ascites, and pleural effusion that resolves after tumor resection. A 53‑year‑old multiparous woman presented with progressive exertional dyspnea and right‑sided pleuritic chest pain. Respiratory and cardiac evaluations were initially unrevealing. Bedside assessment identified mild right basal dullness, and point‑of‑care abdominal ultrasound demonstrated mild free fluid and a solid right adnexal mass. Chest radiography confirmed a small right pleural effusion. Without computed tomography and without diagnostic paracentesis or thoracentesis, Meigs’ syndrome was suspected. The patient underwent laparotomy with total abdominal hysterectomy and bilateral salpingo‑oophorectomy. Histopathology confirmed an ovarian fibroma. Postoperatively, symptoms resolved dramatically, and follow‑up imaging demonstrated complete resolution of the pleural effusion and ascites. This case highlights the importance of considering gynecologic etiologies in unexplained pleural effusion and dyspnea, especially when accompanied by abdominal distension or pelvic pressure.

Atypical Presentation of Lemierre’s Syndrome Masquerading as Gastroenteritis Lemierre’s Syndrome Mimicking Gastroenteritis

Abstract Lemierre’s syndrome (LS) is a rare clinical condition characterized by septic thrombophlebitis of the internal or external jugular vein, usually following an oropharyngeal infection. Here, we present a 20-year-old male patient who developed diarrhea, nausea, and vomiting after receiving clarithromycin for an upper respiratory tract infection. On admission, he had fever, hypotension, and elevation in acute phase reactants (WBC: 20,410/µL, CRP: 197 mg/L). Empirical treatment with ceftriaxone and metronidazole was initiated. Stool and throat cultures were negative. On the second day, abdominal tenderness developed; direct abdominal radiograph showed dilated bowel loops, but toxic megacolon was excluded during follow-up. Thoracic CT revealed septic emboli in the lungs. Due to persistent fever despite ceftriaxone and metronidazole therapy, treatment was escalated to meropenem on the fifth day. On the same day, blood cultures grew Fusobacterium necrophorum, raising suspicion of LS. Doppler ultrasound detected a thrombus in the left external jugular vein. Anticoagulant therapy with low-molecular-weight heparin and clopidogrel was initiated. The fever resolved by the seventh day of full antibiotherapy After three weeks of intravenous therapy, follow-up imaging showed regression of the thrombus. The patient completed a four-week course of antibiotics and anticoagulants and was discharged with full recovery. This case highlights the diagnostic challenge of LS presenting with gastrointestinal symptoms and emphasizes the importance of early blood cultures and imaging. Congruence Between Intended and Observed Transactions in the Implementation of the Senior High School (SHS) Social Studies Curriculum in Ghana

This study aimed to evaluate the Social Studies Curriculum for Senior High Schools in Ghana to determine whether the subject has attained its goal(s) or not, using Stake’s Countenance Evaluation Model. The study took place in the Asante Akim North District. The population for study were all Social Studies teachers and students in the district. The multi-stage sampling technique was adopted for the study and the sample size was 120, made up of 100 students and 20 teachers. The concurrent mixed method approach was adopted. The main instruments used for collecting data were questionnaires, interviews (focus group and unstructured interview), observation and documentary analysis. Quantitative data were tabulated, organised, analysed and interpreted to draw sound conclusions and generalisations. The data were presented in the form of tables and percentages. Words were used to interpret the tables and percentages for easy understanding. The qualitative data responses were described and explained in the form of themes. Sometimes responses were quoted verbatim to authenticate claims made. The study looked at the congruence between what was intended to occur and what was observed before and during the implementation of the curriculum in the Asante Akim North District. On the transactions, it was revealed that teachers in the district communicate the problem the topic seeks to address and the specific objectives to the learners during instruction. Also, teachers varied their teaching methods, techniques and strategies. However, teachers failed to take students out on field trips. Also, they failed to use video documentaries to deliver their lessons inhibiting students’ creativity as they are denied the opportunity to explore and get first-hand information to build up knowledge and develop the needed attitudes and values. From the findings, it is therefore recommended that the National Council for Curriculum and Assessment should increase the time allotted to the teaching of Social Studies at the SHS level from 3-periods of 40 minutes a week to 5-periods of 40 minutes a week to give teachers and learners ample time to discuss, debate, digest and solve problems during instructional hours to enable the goal of the subject attained.

The purpose of the study was to evaluate of Social Studies Teachers Knowledge in Nature, Goals, and Objectives in the Lesson Presentations The study adopted qualitative research approach. Data were collected through the use of individual interviews, observation and focus group discussions for triangulation purposes. Purposive, convenient and simple random sampling techniques were used to select the schools, teachers for the study. In all, seventeen (17) Social Studies teachers were selected from six (6) Senior High Schools. The study revealed that teachers were grossly deficient about the goals of Social Studies. To improve this system, it is recommended that the researchers suggests that the highest priority should be given to the training and development of a new breed of educator who are vocationally competent and academically sound. This can be done by heads of institutions to allow teachers who specialise in their respective areas to teach those subject areas at the various levels of education particularly Social Studies at the High school level.

Assessment of the Effects of Sensors Misalignment of a Multi-Beam Hydrographic Survey

A hydrographic survey vessel shows three -dimensional movements (Roll, Pitch and Heave) misalignment with respect to the vessel reference unit (VRU) due to environmental effects, such as wind, current, other vessel wakes, etc. These motions if ignored, cause errors in measured depth and in the positioning of the sounding. Hence the need of a motion sensor and gyroscope. However, the alignment of the multi-beam sonar head to the motion sensor and gyro (Octant) is critical to the accuracy of the determined depths. It is not possible to install the sonar head in perfect alignment with the motion sensor and gyroscope to the accuracy required. The synchronization of the GPS time with the Motion sensor and gyro, the latency of the position, as reported by the GPS as well as the velocity of sound in water are important parameters to account for the misalignment of the motion senor and the multi beam sonar head; this is called the Patch Test. In view of this, a patch test was done to ascertain the mounting angles of EMB 2058 Multi-beam sonar with Octan V installed onboard a survey vessel (Bitam). The result of the Patch test gives a row, pitch and heading value of -1.242˚, -4.92˚, and -0.48˚respectively. The speed of sound in water as measured ranges from; 1531.47m/s to 1531.60m/s within a minimum cast depth of 0.49m and maximum cast depth of 16.00m. The statistical analysis gives and average error of 2.642cm/m2 which was within acceptable standard as define by the International Hydrographic Organization (IHO).

Aiming at the time complexity of singular value spectrum weighted Hankel SVD filtering algorithm, a clutter suppression algorithm for ultrasonic color Doppler imaging based on BP neural network model is proposed in this paper. Firstly, using the PRF data collected by portable ultrasound instrument, we verify the singular value weighted Hankel SVD filtering algorithm, and the results show that the algorithm has high accuracy; Then, the BP neural network model is established based on the input and output data of singular value weighted Hankel-SVD filtering algorithm; Finally, the clutter suppression algorithm of ultrasonic color Doppler imaging based on BP neural network model is established. The experimental results show that compared with Hankel SVD filtering algorithm, the clutter suppression algorithm proposed in this paper greatly shortens the operation time without reducing the accuracy, so as to improve the real-time performance of the filtering algorithm.

COVID-19 demonstrated a global catastrophe that touched everybody, including the scientific community. As we respond and recover rapidly from this pandemic, there is an opportunity to guarantee that the fabric of our society includes sustainability, fairness, and care. However, approaches to environmental health attempt to decrease the populations burden of COVID-19, toward saving patients from becoming ill along with preserving the allocation of clinical resources and public safety standards. This paper explores environmental and public health evidence-based practices toward responding to Covid-19. A literature review tried to do a deep dive through the use of various search engines such as Mendeley, Research Gate, CAB Abstract, Google Scholar, Summon, PubMed, Scopus, Hinari, Dimension, OARE Abstract, SSRN, Academia search strategy toward retrieving research publications, “grey literature” as well as reports from expert working groups. To achieve enhanced population health, it is recommended to adopt widespread evidence-based strategies, particularly in this uncertain time. As only together can evidence-informed decision-making (EIDM) can become a reality which include effective policies and practices, transparency and accountability of decisions, and equity outcomes; these are all more relevant in resource-constrained contexts, such as Nigeria. Effective and ethical EIDM though requires the production as well as use of high-quality evidence that are timely, appropriate and structured. One way to do so is through co-production. Co-production (or co-creation or co-design) of environmental/public health evidence considered as a key tool for addressing complex global crises such as the high risk of severe COVID-19 in different nations. A significant evidence-based component of environmental/public health (EBEPH) consist of decisions making based on best accessible, evidence that is peer-reviewed; using data as well as systematic information systems; community engagement in policy making; conducting sound evaluation; do a thorough program-planning frameworks; as well as disseminating what is being learned. As researchers, scientists, statisticians, journal editors, practitioners, as well as decision makers strive to improve population health, having a natural tendency toward scrutinizing the scientific literature aimed at novel research findings serving as the foundation for intervention as well as prevention programs. The main inspiration behind conducting research ought to be toward stimulating and collaborating appropriately on public/environmental health action. Hence, there is need for a “Plan B” of effective behavioural, environmental, social as well as systems interventions (BESSI) toward reducing transmission.

We here report a case of an acute cephalic vein thrombophlebitis in a 34-year-old female patient with no known thromboembolic risk factors and no medical history or ongoing treatment. We present the images of her diagnosis, which was made in the presence of a painful and indurated hyperchromic cord of the arm and confirmed by Doppler ultrasound. In the absence of deep extension of the thrombus, his management was limited to a symptomatic treatment without anticoagulation and the symptomatology was amended without complication or recurrence.

The first visualizations of the structures of the sheep's reproductive system, whether of the uterus or ovaries, were performed through exploratory laparotomies, laparoscopies or through materials obtained in slaughterhouse, being possible to evaluate only what was happening at that moment in the estrous cycle. With the emergence of ultrasound from the 90s, it became possible to dynamically monitor what happened at each moment of the estrous cycle at different times of the year, without having to slaughter the females. Because it is a non-invasive technique and the stress caused by its use is very low, it is increasingly necessary to use ultrasound in sheep, either for pregnancy diagnosis or for multiplying herds using biotechnologies, which the ultrasound examination becomes indispensable to obtain the best results. This review aims to describe the main advances in the study of the estrous cycle of sheep with the help of ultrasonography and its influence on the productive efficiency of the herd.

Acoustic Properties of Musanga Cecrepoides Wood Samples Obtained from Different Stem Positions in Niger Delta Region of Nigeria

Abstract Musanga cecrepoides is an interesting wood species due to its diverse utility ranging from medicine, shade, ornaments etc. Research has been conducted on other properties of this wood species with little information about its acoustic properties. Thus, the evaluation of its acoustic properties is pertinent so as to increase the information bank of its properties. This study assessed the acoustic properties of Musanga cecrepoides wood obtained from selected states and locations in the Niger Delta region of Nigerian viz a vis, Rivers, Bayelsa and Delta States. Test samples were collected from different stem positions axially (top, middle, base) and radially (inner and outer) and analyzed using the statistical software package IBM SPSS Statistics, Version 23 (IBM Corporation, New York, USA). Analysis of variance (ANOVA) was performed at 5% level of significance to determine whether the assessed acoustic properties were significantly different among different stem positions. Results showed that sound frequency (f) at top wood (3061.71 Hz) and outer wood (3096.06 Hz) had significantly higher resonance frequency compared with the bottom wood (2768.01 Hz) and inner wood (2349.54 Hz) respectively. The mean sound velocity (v) at both the axial (2069.59 m/s) and radial (1905.96 m/s) stem positions fell short of the estimated mean v of other wood species when compared. However, such result is suitable for other acoustic purposes with moderate sound velocity. The sound radiation coefficient (R) values were highest at the bottom (4.18) axially and outer (4.13) radially when compared to other stem orientations of the wood. To understand the dynamics of M&A, this paper looks at aspects such as targeted firm identification, evaluation, bidding for the target firm, and post-acquisition integration. All forms of M&A, including horizontal, vertical, conglomerate, and acquisitions, are discussed in terms of goals and values, including synergy, cost reduction, competitive advantages, and access to better technology. However, issues such as cultural assimilation, adhesion to regulations, and calculating an inaccurate value are also resolved. The paper then goes deeper to provide insight into how predictive analytics applies to M&A, using ML to improve decision-making with forecasting benefits. Including healthcare, education, and construction industries, the presented predictive models using regression analysis, neural networks, and ensemble techniques help to make decisions. Through time series and real-time data, PDA enables sound M&A strategies, effective risk management and smooth integration.

Predictive modeling, supported by machine learning technology, aims to analyze data in order to guide decision-making towards actions generating desired values in the future. It encompasses the set of techniques used to build models that estimate the value of a certain variable predicting a forthcoming event from the past or current values of relevant attributes. In predictive healthcare modeling, the built models represent the relationship among the data concerning customer, provider, production, and other aspects of the healthcare environment in order to assist the decision processes in the prevention of diseases and in the planning of preventive actions by detection of high-risk patients. Contrary to trend analysis, whose goal is to describe past events, predictive models aim to provide useful indications regarding future events and changes. Predictive healthcare modeling supports actions that try to prevent the manifestation of diseases in healthy individuals or try to diagnose as early as possible the incidence of a disease in patients at risk. A sound predictive analysis encompasses not only the model-training task, but also the aspects of data quality, preprocessing, and fusion during its entire implementation lifecycle to ensure appropriate input data preparation. The robustness of the predictive model and its results depends highly on data quality. Due to the variety of data sources in healthcare environments, it becomes essential to use preprocessing in order to remove noise and inconsistencies. The increasing number of endorsable data exchange standards makes each data exchange achievable, but it demands the implementation of a data-governance program. In addition, the influence of the hospital-database architect on the architecture of an early-diagnosis model is important to guarantee appropriate input-formatting modularity.
