Knowledge Level of Street Fruit Vendors on Food Hygiene in the Tamale Metropolis

This study aimed to assess the knowledge level of street food vendors on hygiene in the Tamale metropolis in the Northern Region of Ghana. The study employed the health belief model as the theoretical basis. Quantitatively, the study employed a descriptive cross-sectional study design to examine the microbial load of street-cut fruits and assess the knowledge and practice of vendors of cut fruits on personal and food hygiene in the study setting. The population consists of cut and vented pawpaw, watermelon, and street fruit vendors registered with the health directorate in the Tamale Metropolis. A convenient sampling technique was used to select 113 respondents for the study. The Yamane formula was used to determine the sample size to select one hundred and thirteen participants (113) out of one hundred and fifty-eight street fruit vendors in the Tamale Metropolis. The main instrument for data collection was a questionnaire. A questionnaire had close-ended questions which were developed using a 'Yes' and 'No' response, and a four-point Likert-type scale ranging from 1=Strongly Disagree (SD), 2=Disagree (D), 3=Agree (A) and 4= Strongly Agree (SA). The data were analysed using descriptive statistics (frequency, percentages, means and standard deviation). The findings revealed that the overall knowledge level of respondents is low. The findings also indicate that vendors do not control the rate at which their customers touch their vended fruits. It is recommended that Street fruit vendors and handlers be educated on fruit hygiene practices through engagement by the Health Directorate Unit of Tamale Metropolis and the Ministry of Health. To keep consumers safe, the Tamale Metropolitan Assembly must strictly enforce compliance with regulations on operation permits and health clearance certificates. Metropolitan sanitation officers must regularly monitor fruit vendors to ensure compliance with goods.

With the rapid growth of online communication platforms, the identification and management of toxic comments have become crucial in maintaining a healthy online environment. Various machine learning approaches have been employed to tackle this problem, ranging from traditional models to more recent attention-based transformer networks. This paper aims to compare the performance of attention-based transformer networks with several traditional machine learning methods for toxic comments classification. We present an in-depth analysis and evaluation of these methods using a common benchmark dataset. The experimental results demonstrate the strengths and limitations of each approach, shedding light on the suitability and efficacy of attention-based transformers in this domain.

Effect of Industrial Effluent on Irrigation Water Quality of Choba River in the Niger Delta Region of Nigeria

Poor irrigation water quality due to oil spillage on surface water can result in food insecurity, health and economic challenges. This paper investigated the effect of total petroleum hydrocarbon (TPHC) and lead (Pb) on irrigation water quality in the oil spill prone area of the Niger Delta region of Nigeria. Water samples were taken from five different sections labelled A, B, C, D, and E along the Choba River, in Rivers State, Nigeria. Sections B, C, D and E were direct industrial effluent discharge points while section A was without direct industrial effluent discharge. Standard methods were employed in the water sampling and analysis. Suitability of Choba river water for irrigation was assessed by comprehensive pollution index (CPI) that incorporated salinity, sodicity, and permeability hazard potentials as well as the specific toxicity hazard potentials of TPHC and Pb. Results showed that all primary water parameters except pH were within the Food and Agriculture Organization (FAO) guidelines. The pH was low, ranging between 4.48 and 5.6. TPHC values for four out of the five samples were greater than the 10mg/l guideline as recommended by the Directorate of Petroleum Resources for surface water. TPHC for the four samples ranged between 14.52 and 174.32mg/l. The parameters with the most impact on CPI include EC, PI and TPHC with TPHC having the most impact. Water samples from sections A, B and E with CPI values 0.14, 0.37, and 0.8 respectively were classified in the clean, sub clean and slightly polluted categories respectively, while water samples from sections C and D with CPI values greater than 1 range from moderately to heavily polluted and not suitable for agricultural irrigation. Only water sample A was found suitable for irrigation.

Background: Currently, the DRL quantity in mammography are evaluated in terms of mean glandular dose (MGD). Since the MGD cannot be measured directly, it can be obtained by calculation using the equation (D=K*g*c*s). In previous studies, the conversion factor g was calculated by Monte Carlo simulation and is not reported from actual measurements. In this study, we focused on the g-factor, which is a conversion factor to the MGD at 50% glandularity, and attempted to measure it using a nanoDot dosimeter to see if it can be used in mammography. Methods: The nanoDot dosimeters were inserted in a PMMA phantom at depths ranging from 0 cm to 6 cm in 1 cm increments, and measurements were made in three HVLs of 0.3 mmAl, 0.35 mmAl, and 0.4 mmAl HVL. The g-factor was calculated from the nanoDot dosimeter values using a conversion equation. Results and Discussion: The measured g-factors for all the HVLs were in close agreement with those of Dance et al. The values of the previous studies did not include the backscatter factor, which may have underestimated the MGD. The difference was smaller for the 0.4 mm Al. Compared to the other HVLs, the 0.4 mm Al was measured without a compression plate, which may have been influenced by the presence or absence of a compression plate. Conclusion: The nanoDot dosimeters were used to calculate g-factors. The results agreed with those of previous studies within uncertainty. This indicates that nanoDot dosimeters can be used in the mammography field.

Herein we introduce a reliable and effective method, allowing any musician, regardless of the theoretical background, to carry out a 4-way jazz harmonization of whatever melodic progression almost instantly, with few exceptions. Many jazz students experience a deep frustration in dealing with the harmonization of non-diatonic notes. Sometimes, moreover, a coherent harmonization of the aforementioned notes can turn out to be a very challenging task even for extremely skilled professionals. In this paper, the harmonization of the chromatic scale in different harmonic contexts is accurately discussed, by resorting to the well-known concepts of harmonic functions, tonicization, chromatic and diatonic parallelism, and auxiliary chords. All the chords are labelled so as to allow the reader to immediately understand their role in the particular harmonic context. Consequently, the procedure essentially translates into an optimization of the "harmonic flow".

This study proposes an integrated approach combining geographic information systems (GIS) and geotechnical analyses to assess the stability of the Oued Ayda mountain lake dam, located in the Siliana Governorate, northwestern Tunisia. The mechanical properties of the embankment and foundation materials were integrated into a Mohr-Coulomb geomechanical model, while the pore water pressure distribution was simulated for various representative hydromechanical scenarios: end of construction, normal operation, rapid drainage, and short- and long-term empty reservoir conditions. The stability analysis, performed using the Morgenstern-Price method with the SLOPE/W software (GeoStudio), reveals high safety factors on the upstream side (SF > 3 in the short term and SF ≥ 2 in the long term), indicating good resistance of this slope to hydraulic and mechanical stresses. Conversely, the results show that the downstream slope exhibits significantly lower safety factors, ranging from 1.335 to 1.338 under long-term conditions, particularly during normal operating and rapid drainage scenarios. These reduced values indicate a high vulnerability of this slope to persistent saturation and adverse hydraulic gradients. In conclusion, although the dam exhibits satisfactory overall stability, the downstream slope remains the most vulnerable area of the structure. The results underscore the need for rigorous management of water level fluctuations and suggest reinforcing the drainage system or implementing targeted stabilization measures to ensure the long-term safety and durability of the structure.

VAP, or Ventilator Associated Pneumonia, is a type of pneumonia that arises in patients receiving mechanical ventilation. This condition is a serious complication and can lead to the patient's decline while on a mechanical ventilator, posing a significant risk for secondary complications if not addressed promptly. In particular, VAP is a prevalent issue in intensive care units, where the healthcare team works to prevent further deterioration of the patient. VAP is associated with a notably high mortality rate, particularly in individuals with weakened immune systems, as well as in younger and older populations. Extended intubation and prolonged sedation can contribute to the onset of VAP. The previous study had found that incidence of VAP accounts for 9 % to 27 % endotracheal intubated patients, whereas VAP has a mortality rate ranging from 25 % to 50 % [1-3]. VAP increased ICU and hospital length of stay, antibiotic consumption, and healthcare cost (Zhi Mao et al, 2016) [4]. While using the subglottic suctioning could prevent and help the intubated patient to decreased the development of VAP. The study shown by Smith et al (2021) [5] SSD is a technique employed to reduce micro aspiration of oropharyngeal secretions in patients with cuffed endotracheal airways. Aspiration of oropharyngeal secretions is the accepted cause of the majority of ventilator-associated pneumonia (VAP), a complication of invasive ventilation with high associated mortality. Another study by Rahul Gujadhur et al (2005) [6], subglottic suction has also been shown to delay the onset of VAP but no benefits in terms of ventilation time, hospital stay or mortality benefit have ever been shown. This investigation into subglottic suctioning may assist the healthcare team, particularly in the intensive care unit, in preventing the development of VAP and shortening the duration of ventilation for patients. The advantages of subglottic suctioning are often debated, particularly regarding its impact on the duration of intubation. However, recent research indicates that implementing subglottic suctioning within a time frame of fewer than three days could help reduce intubation duration and lead to improved patient recovery.

Handling Practices of Folded Vermicelli by Small-scale Processors in Tanga City, Tanzania

This study assessed the handling and processing practices of 30 small-scale folded vermicelli processors in Tanga, specifically in urban areas of Tanga City, Tanzania. However, the micro- and small-scale processors were producing in unhygienic way because they are lacking facilities and equipment to process and handle the product hygienically. Multistage sampling design was adopted for this study and face-to-face interviews were conducted to collect data from all processing units through nine streets using semi-structured questionnaires and observation checklists. Data were analyzed using Statistical Package for Social Sciences, where the statistics aspect was determined from the results obtained. The processors found across various streets (ranging from 3.3% in Kwaminchi Street to 23.3% in Mabawa Street), exhibited diverse demographics, with 53.3% being owner-operators and 40% and 6.7% in labourer and supervisor roles, respectively. A significant portion (53.3%) had 1-3 years of experience, and a small portion (10%) attended formal training in pasta processing. Despite 73.3% possessing food manufacturing licenses, many were unfamiliar with legal requirements, lacking documentation and standardized processes, raising concerns about food safety. Raw materials were sourced locally, but 56.7% lacked storage facilities. Hygienic practices varied, with 43.3% undergoing periodic medical check-ups, 70% using protective gear, and 60% had hand washing facilities. Sun drying was the sole method employed, with 86.7% placed drying trays on rooftops. Packaging practices raised concerns, as 93.3% reused woven polypropylene bags, potentially impacting product quality. Awareness of aflatoxin and its health implications was lacking in 90% of the processors. Overall, the study highlighted gaps in awareness, training, and adherence to standards among processors, posing potential risks to food safety and quality. Encourage them to adhere with Tanzania Bureau of Standards requirements and formalize their quality control practices.

Background: In the landscape of education, the decision-making process that leads students to pursue or reject nursing as a career is a multifaceted phenomenon shaped by a plethora of influences ranging from personal experiences to societal norms. Aim: To explore non-nursing students' insights on nursing education, seeking to shed light on the considerations and challenges that influence their views on nursing education. Materials & Methods: A qualitative approach using thematic analysis were utilized. Lincoln and Guba's framework for rigor and trustworthiness directed the validation process. Semi-structured interviews based on vetted questionnaires yielded the data. Results: Analysis of interviews with ten (10) non-nursing college students revealed three key themes: 1) initial insights, 2) factors influencing their insights, and 3) difficulty of nursing education. Non-nursing students view nursing education as multifaceted and rigorous, recognizing the profession's complexity but have reservations about the heavy workload, intense clinical demands, and health risks, particularly highlighted by the pandemic, which contributes to their reluctance to choose nursing as a career path. Implications: Addressing perceptions, enhancing curricula, offering mentorship, and providing emotional support, nursing education can be improved, steering more students towards a career in nursing. Conclusion: Non-nursing students respect the complexity of the nursing profession but are deterred by its demands and risks, indicating a need for educational reforms to better convey the role, value, and opportunities within nursing to encourage more students into the field.

Optimization of Delirium Care in Adult Patients with Cancer: A Comprehensive and Integrative Review of Efficacy and Patient Outcomes

Delirium is a major complication most commonly observed in patients with advanced cancer. However, despite its prevalence, the early diagnosis, management, and prevention of this condition have not seen significant progress. Aim of this research is to provide insights into the prevalence of delirium, the optimization of interventions for managing delirium symptoms, their effectiveness and the impact of underlying factors on the reversibility of delirium in advanced cancer patients receiving palliative care. The review involved systematic searches of relevant databases including MEDLINE, CINAHL, ProQuest Nursing and Allied Health, and PsychInfo using refined search terms. Eight publications out of 614 studies originally searched were selected and critically reviewed. Their quality was assessed using Joanna Briggs Institute's Critical Appraisal Tool for Case Series. Data abstraction and content analysis were performed to synthesize the findings. Delirium is prevalent among advanced cancer patients in palliative care, with rates ranging from 10.3% to 24.1%. Pharmacotherapy and non-pharmacological interventions showed effectiveness in reducing delirium symptoms. Delirium was found to be reversible through palliative care interventions, antipsychotic medications, and exercise therapy. Effective delirium management is crucial in improving the quality of life of cancer patients. This review emphasizes the importance of subtype-specific treatments, standardized guidelines, and long-term follow-up studies. Implementing evidence-based individualized approaches to delirium management can optimize treatment efficacy and clinical outcomes in patients as well as improve the quality of care. Tailored interventions, standardized protocols, and further research are hereby recommended.

Although human oral fluid has become more routine for quantitative drug detection in pain management, detecting a large scope of medications and substances is costly and technically challenging for laboratories. This paper presents a quantitative assay for 64 pain medications, illicit substances, and drug metabolites in human oral fluid. The novelty of this assay is that it was developed on an older model AB SCIEX 4000 instrument and renders obscure the need for more technical and expensive laboratory equipment. This method includes addition of internal standard and a 2-step liquid-liquid extraction and dry-down step to concentrate and clean the samples. The samples were suspended in 50% MeOH in water and separation and detection was accomplished using triple quadrupole mass spectrometry (LC-MS/MS). Separation was achieved using reverse-phase liquid chromatography with detection by LC-MS/MS. A second injection was done in negative mode to determine THC-COOH concentration as an indicator of THC. An aliquot of the (already) extracted samples was analyzed for D- and L- isomers of amphetamine and methamphetamine using a chiral column. The standard curve spanned from 5 to 2000 ng/mL for most of the analytes (1 to 2000 ng/mL for fentanyl and THC-COOH) and up to 1000 ng/mL for 13 analytes. Pregabalin and gabapentin ranged from 25 to 2000 ng/mL. The result is a low-cost method for the sensitive detection of a wide-ranging oral fluid menu for pain management. This assay has a high sensitivity, and good precision and accuracy for all analytes with an older model mass spectrometer.

Haematological Alterations among Patients with Oral and Dental Health Problems in the Tripoli Region

Abstract Background: Oral health plays an important role in maintaining life functions and quality of life. Gingivitis and periodontitis are two forms of periodontal disease that exist on a spectrum ranging from bleeding, erythematous, and inflamed gingival tissue to the loss of attachment and alveolar bone. Dental caries and periodontal diseases are the most common oral ailments and the major causes of tooth loss. Objectives: The study aimed to evaluate the haematological alterations among patients with oral and dental health problems in the Tripoli region. Material and Methods: The present study was conducted on 200 patients with oral and dental health problems attending six medical centers in Tripoli region from the 01st March 2022 to the 01st June 2022. Also, 100 healthy individuals without any oral and dental health problems or any other diseases were recruited as a control group. This study was approved by the Research and Ethical Committee of the Libyan Academy of graduate studies and medical centers. One ml of venous blood was withdrawn from each participant in the study for the determination of haematological parameters. The data were compared using SPSS Statistics for Windows, Version 26. Results: The results showed that red blood corpuscles (RBCs) count, hemoglobin concentration, hematocrit value, MCV, MCH, and MCHC in patients with oral and dental health problems were decreased significantly (P<0.01) compared to the control group. White blood cell count, neutrophils%, and platelets count in patients with oral and dental health problems were significantly (P<0.01) increased compared to the control group. Paper is widely used in packaging applications and is biodegradable and therefore perfectly safe as green packaging wrap for the environment. The hydrophilic nature of cellulose fibrils limits the water vapour and oxygen barrier properties of paper. To mitigate these limitations, paper is often associated with other materials, such as plastics, wax and aluminum, for achieving their good barrier properties. However, these materials suffer from serious environmental issues, as difficult and inefficient to recycle. Recently, cellulose nanofibre (CNF) based materials has been considered as an alternative to produce eco-friendly barrier materials. Existing techniques to prepare cellulose nanofibre films/sheets/composites/ laminates on the paper substrates are commercially not feasible and expensive. Therefore, other cost effective and readily implementable methodologies are required to achieve cellulose nanofibre barrier layers. In the present report, a novel approach is developed using spray coating technique to produce CNF materials with excellent barrier properties. Among many coating techniques, the spray coating has many advantages such as the production of even coating surface on the base sheet and contactless coating with the substrate. A laboratory scale spray coating of cellulose nanofibre suspension on a paper substrate was developed. When the cellulose nanofibre suspension concentration was varied from 0.5 to 1.5 wt. %, coat weight is increased from 2.9±0.7 to 29.3±6.9 g/m2. As a result, the air permeability of composite was decreased 0.78±0.17 to <0.0030 µm/Pa.s. Scanning electron microscopy studies of spray coated CNF laminates on the paper confirms that the surface pores in the paper substrates are filled with sprayed cellulose nanofibre and forms a continuous film on the surface of the substrate. These are the probable reasons for the reduction of air permeability of composites. A rapid preparation technique to prepare free standing cellulose nanofibre films/sheets was also developed using a bench scale spray coating system. Cellulose nanofibre suspension with concentration ranging from 1 to 2 wt% was sprayed onto a stainless steel plate, which is moving on a conveyor at a velocity of 0.32 cm/sec and then air dried. The basis weight of produced cellulose nanofibre films is varied from 52.8±7.4 to 193.1±3.4 g/m2. Processing time taken to prepare films was approximately 1.0 min, which is much less than processing times reported in the previous literature. Thus, the significant reduction in preparation time for producing the cellulose nanofibre sheet recommends that this spray coating technique can be utilized for the development of a scalable process for the fabrication of various cellulose based nanocomposite. Therefore, the laboratory scale spray coating confirms that the spraying could provide a platform for development of films/sheets/nanocomposite and also a CNF barrier layer on the base sheet. The future work is the development of a continuous spray coating of cellulose nanofibre on the base sheet and evaluation of mechanical and barrier properties spray coated barrier layers on the base sheet.

Assessment of Microbiological Quality of Ready to Eat Food Served in Ships Along Warri, Koko and Port Harcourt Water Ways, Nigeria

Abstract Background: Food borne outbreaks have been associated with sourcing unsafe food. Therefore, the first preventative strategy should be to source safe food. Even if the sourced food is safe, measures need to be put in place to ensure that it remains safe during the transfer, storage, preparation and serving activities that follow. An understanding of the ship food supply and transfer chain will help to illustrate the points at which the food can become contaminated en route to the point of consumption. Objectives: The study was conducted in selected sea port in the core Niger Delta to assessed the microbiological quality of food served at different ship galley to crew and passengers and compered it to standard. Methods: Samples of food were taken from three (Port Harcourt Area one (PHSP), Warri (WSP) and Koko (KSP)) seaports within the South-South zone for laboratory analysis to uncover food spoilage microorganisms capable of causing disease outbreak among ship which could result to Trans border diseases. Eleven samples of different ready to eat food were collected from the locations, which included cooked rice; fried fish, irish potato porridge, vegetable soup, griki, pepper soup, fried irish potato, salad and bread were collected randomly. The samples were prepared and analyzed using standard procedures. Mean viable counts of aerobic and anaerobic bacteria were determined, ranging from (13×103cfu/g to 78×104 cfu/g) for ready to eat food. Mucormycosis and Candida Infections in Patients of COVID-19 Pneumonia: A Systematic Review

Introduction: The city of Wuhan in China reported the first case of coronavirus, termed as SARS-CoV-2, in December 2019. To date, 187,827,660 cases have been reported to the WHO (3). With current research focusing on potential therapeutic agents for the coronavirus disease and vaccines, there remain major gaps in our understanding of the pathophysiology and clinical course of this viral pneumonia. Secondary infections are one of them. In this systematic review, we analyze the outcomes of two fungal infections in patients of COVID-19, viz. Mucormycosis and candida. Methodology: A systematic review has been done on secondary infections with mucor and candida fungi in patients of COVID-19. Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines were used. Twenty-three studies were included in the final analysis. Our review included studies from various countries across the globe. The risk of bias was analyzed using the NIH Quality Assessment Tool for Case Series Studies. This study did not require ethical approval as data was obtained from already available databases, and patients were not directly involved. Results: A total of 23 articles were included in the final review and the total number of patients included was 79 Male: female ratio was calculated to be 1.6 and the average age of patients was 52 years (ranging from 24-86 years). Various types of comorbidities were seen in the included patients, the most common being diabetes mellitus. Among the 18 patients in the cohort of mucormycosis, 7 patients died and four studies did not report patient outcomes. Among the 61 patients, 13 patients died and one patient was still ventilated at the time of publication. Conclusion: Secondary infections after COVID-19 are a cause of major concerns. Further studies and case reports are needed to better understand the various other types of secondary infections and also to formulate strategies to prevent these.

Native Middle Egypt Rabbit breed (NMER) was crossbred with Gaint Flander rabbits to create a synthetic line. This study was aimed to evaluate the genetic estimates of this synthetic line with comparing to the purebreds. A crossbreeding was carried out by mating bucks of Gaint Flander (G) with does of NMER (N) to get F1 (½N½G), then does and bucks of F1 were mated to get F2 (½N½G)2, followed by two generations of inter se-mating to get a new synthetic line is called Egy-line with a genetic structure of ((½N½G)2)2. Heritability estimates for body weights were generally moderate and ranged from 0.10 to 0.24, while the estimates of heritability for growth rate were low and moderate and ranging from 0.01 to 0.23. Common little effects of body weight were large as weaning (0.61), then declined gradually as the rabbit grew older. Also, the same trends were observed for relative growth rate (RGR). The direct additive effects were positive and highly significant for all body weights at different ages, favoring Gaint Flander and heavier comparing with NMER rabbits. Most relative growth rates during different intervals were non-significant. Gaint Flander was highly significant and heavier in maternal additive effects it in different weeks of age comparing with NMER rabbits. Direct heterosis effect for most bodyweight was positive and highly significant, and percentages of direct heterosis increased generally with the advance of age. Maternal heterosis for growth rates from 5 to 6, 8 to 10, and 10 to 12 week was positive, only. Direct recombination effects for most bodyweight were positive and highly significantly exclude weight at 5 and 6 weeks. It is concluded that a new synesthetic line (Egy-line) has proven its superiority and performance well in all different body weights and most growth rates compared to other parents and crossbreds.

Acoustic Properties of Musanga Cecrepoides Wood Samples Obtained from Different Stem Positions in Niger Delta Region of Nigeria

Abstract Musanga cecrepoides is an interesting wood species due to its diverse utility ranging from medicine, shade, ornaments etc. Research has been conducted on other properties of this wood species with little information about its acoustic properties. Thus, the evaluation of its acoustic properties is pertinent so as to increase the information bank of its properties. This study assessed the acoustic properties of Musanga cecrepoides wood obtained from selected states and locations in the Niger Delta region of Nigerian viz a vis, Rivers, Bayelsa and Delta States. Test samples were collected from different stem positions axially (top, middle, base) and radially (inner and outer) and analyzed using the statistical software package IBM SPSS Statistics, Version 23 (IBM Corporation, New York, USA). Analysis of variance (ANOVA) was performed at 5% level of significance to determine whether the assessed acoustic properties were significantly different among different stem positions. Results showed that sound frequency (f) at top wood (3061.71 Hz) and outer wood (3096.06 Hz) had significantly higher resonance frequency compared with the bottom wood (2768.01 Hz) and inner wood (2349.54 Hz) respectively. The mean sound velocity (v) at both the axial (2069.59 m/s) and radial (1905.96 m/s) stem positions fell short of the estimated mean v of other wood species when compared. However, such result is suitable for other acoustic purposes with moderate sound velocity. The sound radiation coefficient (R) values were highest at the bottom (4.18) axially and outer (4.13) radially when compared to other stem orientations of the wood. The management of supply chains is increasingly complex. This study provides a comparative analysis of the cost-benefit analysis for managing various risks. It identifies the financial implications of leveraging artificial intelligence in supply chains to better address risk. Empirical results show a business case for managing some sources of risk more proactively facilitated through predictive modeling techniques offered by AI. Across investigation streams, the use of AI results in an average total cost saving ranging from 41,254 to 4,099,617. Findings from our research can be used to inform managers and theorists about the implications of integrating AI technologies to manage risks in the supply chain. Our work also highlights areas for future research. Given the growing interest in studying sub-second forecasting, our research could be a point of departure for future investigations aimed at considering the impact of forecasting horizons such as an intra-day basis. We formulate a conceptual framework that considers how and to what extent performance evaluation metrics vary according to differences in the fidelity of predictive models and factor importance for identifying risks. We also utilize a mixed-method approach to demonstrate the applicability of our ideas in practice. Our results illustrate the financial implications of integrating AI predictive tools with business processes. Results suggest that real-world companies can circumvent inefficiencies associated with trying to manage many classes of risk via the use of AI-enhanced predictive analytics. As managers need to justify investment to top management, our work supports decision-making by providing a means of conducting a trade-off analysis at the tactical level.

Data warehousing is a technique for collecting, managing, and presenting data to help people analyze and use that data effectively. It involves a large database designed to support management-level staff by providing all the relevant historical data for analysis. This chapter begins with a definition of data warehousing, followed by an overview of large-scale policy management to highlight the need for data warehousing. Next, an overview of an ETL framework is presented, along with a discussion of advanced ETL techniques. The chapter concludes with an outline of performance optimization techniques for data warehousing. Data warehousing is considered a key enabler for efficient reporting and analysis, with implementation choices ranging from cost-effective desktop systems to large-scale, mission-critical data marts and warehouses containing petabytes of data. Extract, transform, and load (ETL) systems remain one of the largest cost and effort areas within data warehouse development projects, requiring significant planning and resources to build, manage, and monitor the flow of data from source systems into the data warehouse. The technology and techniques used for ETL can greatly influence the success or failure of a data warehouse. Complex business requirements for data cleansing, loading, transformation, and integration have intensified, while operational plans for real-time and near-real-time reporting add additional challenges. Parallel loading mechanisms, incremental data loading, and runtime update and insert strategies not only improve ETL performance but also optimize data warehousing performance, particularly for large-scale policy management.

Leveraging AI, ML, and Generative Neural Models to Bridge Gaps in Genetic Therapy Access and Real-Time Resource Allocation

This paper leverages gene and cell therapy research in diverse disorders ranging from monogenic to infectious diseases to cancer and emerging breakthroughs, where one can harness individual genes or a synthetic gene sequence designed based on a shared molecular pattern in infected cells to better fight various disorders [1]. A pivotal task is to predict the performances of candidate gene therapies to guide clinical translational research using methods such as retrospective bioinformatic analyses. Implementing them to a large-scale gene therapy database reveals that it is feasible to construct and apply well-performing interpretable, supervised learning models [2]. Preliminary evidence of machine learning approaches' statistical significance helps clinicians and biomedical researchers, market participants, and regulatory and economic experts derive relevant, practical applications, thereby enhancing the deployment of gene therapy and genomics to achieve positive, long-term growth for humanity while alleviating the ongoing worldwide economic burden precipitated by prolonged and recurring diseases. Deploying machine learning techniques to accelerate gene and cell therapy drug development and trials shall also mitigate the existing obstacle of limited patient access to emerging, transformative medical innovations such as gene therapy due to skyrocketing prices, which often herald gene therapy products as the world's most expensive medicines [3]. Moreover, in preventing patients from accessing effective, life-saving genetic medicines, there commonly exists a multidimensional access gap encompassing the availability, affordability, and quality or acceptability of these clinical treatments. The ensuing substantial gap has repeatedly been documented and mainly emanates from differential institutional and socio-political choices around resource allocation at international and domestic levels [4]. Particularly, it is also due to the stringent licensure and regulatory approval processes underpinned by insufficient evidence for novel safety and clinical efficacy profiles for genetic therapies in multiple micro-local diagnoses and subpopulations. We believe that a higher likelihood of gene therapy adoption shall result when the clinical evidence path contains adequate representation from the most diverse and relevant patient populations [5].
